We are hiring for Lead Physical Therapist to join our team in Senior Living.

• Documents all Outpatient and HH visit types per company standards.
\n
• Timely initiation of care for Outpatient and HH SOC.
\n
• Schedules patient at appropriate time for therapy.
\n
• Identifies patient and family education needs.
\n
• Collaborates with the team to establish interdisciplinary team goals.
\n
• Utilizes equipment in a proper and safe manner, reports broken or faulty equipment.
\n
• Follows medical regimen developed for the patient. Is aware of and adheres to precautions (THR, TKR,
  Cardiac, Wound, etc.).
\n
• Utilizes the following safe treatment techniques: transfers, positioning, feeding, splinting, paraffin bath,
  cryotherapy, etc.
\n
• Utilizes treatment approaches according to established PT protocols and policies.
\n
• Utilizes alternatives or other innovative approaches as needed.
\n
• Processes and secures physician orders.
\n
• Aware of financial situation and plans care appropriately; ensures that patient is aware of home/work place
  accessibility; ensures patient awareness of needed equipment, and makes plans to secure it; designs a home
  exercise program to address all pre-existing problems; makes all appropriate referrals at D/C.

• Serves as a member of the Performance Improvement team
\n
• Actively participates in Department Team meetings.
\n
• Conducts in services for other staff members as needed or identified.
\n
• Participates in weekend and holiday rotation as needed
\n
• Participates in student supervision as indicated.
\n
• Assists in the interview process for new therapy department employees as needed.
\n
• Ensures all therapists perform treatments and documentation that meet all company standards.
\n
• Attends Team Conference weekly and functions as an active participant of the ID team.
\n
• Proficient in Wound care as allowed by state practice acts.
\n

\n
• Must be a currently licensed Physical Therapist.
\n
• CPR certification
\n
• Current Physical Therapy License in state of practice
\n
• Membership or certification in national association (as applicable)
\n
• Must be willing to accept management duties as assigned
\n
