for the BA Case Manager
Provides case management services to clients, including but not limited to transportation, referral, vocational and psychosocial education, and linkage with community supports.
Essential Job Functions for success in the Case Management role:
- Provide direct client services and case management to BHG clients.
- Interface with other community agencies and resources to optimize service to BHG clients.
- Identify community resources and facilitate client referrals.
- Work directly with BHG clients and their family, friends, and partners to provide psychosocial support.
- Effectively present San Luis Valley Behavioral Health Group mission to San Luis Valley communities.
- Serve as a liaison and resource to San Luis Valley community agencies and area educators concerning persons with mental illness.
- Effectively communicate with other BHG personnel regarding client needs.
- Is responsible for record-keeping and reports as may be required by Center administration, and to subscribe to the code of ethics of this discipline.
- Utilizes Center technology for timely record keeping and report-writing.
- Meets performance standards as specified by supervisor and management staff.
Secondary Job Functions
- Work closely with BHG management and personnel to create new client service programs and evaluate those existing programs.
- Assist in providing or arranging for individual, family, couple or group counseling
Requirements:
Education/Skill Requirements
Education & Experience: Minimum of Bachelor's degree in social work, psychology or related field. 2+ years of experience in case management or a related role. Should be able to obtain QMAP certification within 90 days of hire. Driving is required for the role.

- Basic Computer Skills and ability to properly document services
